Entropion
Diagnosis, ocular-surface protection, surgery and the possible temporary use of botulinum toxin in selected cases.
In entropion, the eyelid margin—most often the lower eyelid—turns inwards. The eyelashes may then come into contact with the conjunctiva and cornea.
Characteristic sign: eyelashes turned towards the eye, with irritation, watering or a foreign-body sensation.
Why it may cause discomfort
Continuous rubbing by the eyelashes can disturb the ocular surface and cause:
- a foreign-body sensation;
- burning or pain;
- watering;
- redness;
- discharge;
- sensitivity to light;
- blurred vision when the cornea is irritated.
If persistent, eyelash contact can cause corneal abrasions or injury and should not be ignored.
Assessment and treatment
The approach depends on the cause, eyelid position and condition of the ocular surface. A doctor may temporarily recommend protective or lubricating measures; in appropriate cases, surgical correction may be proposed.
In selected cases of involutional entropion, particularly where there is a spastic component, botulinum toxin may be considered as a temporary option. Its use depends on eyelid examination and does not automatically replace surgical correction.
The technique is not selected from appearance alone, but after evaluating the tissues and the mechanism causing the eyelid to turn inwards.
When urgent assessment is needed
Significant pain, marked redness, sensitivity to light or reduced vision may indicate corneal involvement and require urgent ophthalmic assessment.

Can entropion damage the eye?
Yes. Continuous eyelash contact with the cornea can cause abrasions or more serious injury.
Is surgery always necessary?
Not in every case. The decision depends on the cause, duration, symptoms and risk to the ocular surface.
Can botulinum toxin be used for entropion?
In selected cases it may be considered as a temporary measure. It is not suitable for every form of entropion and does not automatically replace surgical treatment.
Can I simply remove the eyelashes that are causing discomfort?
This does not necessarily correct the abnormal eyelid position. It is important to have the cause assessed.
